PATH serves patients who have been discharged from a hospital but who still require therapy and special skilled care in order to make the transition from hospital to home. Wichita Presbyterian Manor has a track record of successful rehabilitation outcomes. By incorporating our team of medical professionals to develop individualized programs, PATH helps patients regain functionality and promotes a safe return to their home.
Most of the patients admitted to PATH follow the Medicare guidelines for skilled care. In order for the stay in PATH to be covered under the Medicare program, the patient must have a 3-day qualifying stay in an acute care hospital or have been discharged from a 3-day qualifying stay within 30 days prior to admission to PATH.
Our PATH team assesses all patients referred to our program while they are still in the hospital to ensure they are an appropriate admission to PATH and gathers the following information:
- Prior level of function
- Support system
- Attitude toward recovery
- Medical stability and
- Ability to tolerate intensity of care.

Steps to Admission
Inform your hospital Social Worker that you have selected Wichita Presbyterian Manor for your rehabilitation stay.
The hospital Social Worker will contact Wichita Presbyterian Manor on your behalf to check availability and start admissions. She or he will work directly with our program director to facilitate the process. She or he will guide you through the hospital discharge and arrangements to be admitted to PATH Rehabilitation.
